FAPI Therapy for various Cancers

Summary in a Minute

FAPI therapy is a targeted treatment that uses radiolabeled fibroblast activation protein inhibitors (FAPI) to treat certain cancers, including solid tumors. FAPI binds to fibroblast activation protein, which is overexpressed in many cancer types, including pancreatic, gastric, and lung cancer. This nuclear medicine therapy delivers targeted radiation to tumor cells, minimizing damage to surrounding healthy tissue. FAPI therapy in India is an emerging approach, offering new hope for patients with advanced cancers that may not respond well to conventional treatments.

Overview

FAPI therapy is a targeted radiation therapy in India for various cancers, utilizing small molecules that bind to Fibroblast Activation Protein (FAP) inside tumor microenvironment.

Mechanism of Action

1. FAP-specific peptides bind to FAP-expressing cancer-associated fibroblasts (CAFs).
2. Radionuclide (e.g., Lutetium-177 or Actinium-225) conjugated to peptide delivers radiation to
tumor microenvironment.
3. CAFs are eliminated, disrupting tumor stroma and enhancing anti-tumor immunity.

Indications

Any FAPI positive advanced cancer (determined on a pre-treatment FAPI PET scan) with limited treatment options
In our experience, we have treated following cancers with encouraging results
1. Sarcoma
2. Pancreatic cancer
3. Breast cancer
4. Non-small cell lung cancer
5. Colorectal cancer
6. Ovarian cancer
7. Desmoid tumor
8. Gall bladder cancer
9. Cholangiocarcinoma
10. Buccal mucosa cancer

Contra-Indications

1. Pregnancy or breastfeeding.
2. Severe kidney impairment.
3. Known hypersensitivity to FAPI compounds.

Dosage and Administration

Can be combined with Lutetium-177 PRRT Therapy (Radioactive isotope emitting beta rays for tumor cell killing) or Actinium-225 PRRT Therapy (Radioactive isotope emitting alpha rays for tumor cell killing)
For choice of radioisotope, dosage, number of cycles and other treatment related information please connect with us.

Efficacy

1. Improved progression-free survival.
2. Reduced tumor burden.

3. Alleviation of symptoms.
3. Enhanced quality of life.

Side Effects

1. Fatigue.
2. Nausea and vomiting.
3. Dry mouth (with Actinium-225).
3. Bone marrow suppression (Rare).
4. Kidney damage (Rare).

Radiation Safety

1. Standard radiation safety protocols.
2. Patient isolation (24-48 hours).

Therapeutic Applications

1. Monotherapy: FAP-targeted radionuclide therapy.
2. Combination therapy: FAPI Therapy in Delhi NCR, India combined with chemotherapy, immunotherapy, or
targeted therapies.
3. Sandwich Therapy: Alternating Lutetium-177 & Actinium-225 FAPI Therapy

FAQs about FAPI Therapy

FAPI therapy targets cancers with fibroblast activation protein, including pancreatic, gastric, and lung cancers.

FAPI binds to cancer cells, delivering radiation directly to tumors, minimizing harm to healthy tissues.

Yes, FAPI therapy is generally well-tolerated, though side effects like fatigue or nausea may occur.

The treatment typically involves a single injection followed by imaging, taking 1-2 hours in total.
