I-131 Therapy for Thyrotoxicosis

Summary in a Minute

I-131 therapy is a common treatment for thyrotoxicosis, particularly in conditions like Graves’ disease or toxic nodular goiter. The therapy involves administering radioactive iodine (I-131) orally, typically in capsule or liquid form. The thyroid gland absorbs the iodine, which delivers targeted radiation to destroy overactive thyroid tissue. Depending on the dose, this may eliminate part or all of the thyroid tissue, reducing thyroid hormone production. I-131 therapy provides long-term relief from hyperthyroid symptoms, with minimal side effects and a high success rate in managing thyrotoxicosis.

Overview

I-131 therapy involves administering radioactive iodine orally, usually in capsule or liquid form. The ingested radioactive iodine is selectively taken up by the thyroid gland, where it destroys part or all of the thyroid tissue, depending on the dose.

Indications

1. Graves’ disease (especially after failed antithyroid medication or surgery)
2. Toxic multinodular goiter (Toxic MNG)
3. Solitary toxic nodule (Autonomously functioning thyroid nodule or AFTN)
4. Hyperthyroidism due to thyroid cancer

Procedure

1. Pretherapy nuclear thyroid scan is essential to plan I-131 therapeutic dose
2. Preparation: Stop antithyroid medications 4-5 days before treatment
3. Administration: Swallow the radioactive iodine capsule or liquid
4. Follow-up: Monitor thyroid function tests (TSH, Free T4) 1-3 months post-treatment
5. Adjustments: May require additional treatments or medication adjustments

Contra-Indications

1. Pregnancy or breastfeeding
2. Severe thyroid-associated ophthalmopathy (Graves’ Ophthalmopathy)
3. Uncontrolled thyroid crisis

Benefits

1. High success rate (up to 90%): I-131 therapy effectively cures thyrotoxicosis in up to 90% of cases.
2. Non-invasive: I-131 therapy requires no surgery, just an oral radioactive iodine dose.
3. Low risk of complications: Minimal side effects occur, with rare long-term complications or side effects.
4. Mostly an outpatient procedure: The treatment typically involves a single visit, requiring no hospitalization.

Side Effects

1. Radiation safety precautions (e.g., avoiding contact with pregnant women, children)
2. Risk of hypothyroidism (underactive thyroid)
3. Potential for thyroid gland swelling or tenderness

Post-treatment Considerations:

1. Regular thyroid function monitoring
2. Potential need for lifelong thyroid hormone replacement therapy (levothyroxine)
3. Avoid pregnancy for 6-12 months after treatment

It’s essential to consult with a nuclear medicine specialist and an endocrinologist to discuss the specifics of I-131 therapy and determine if it’s the right treatment.

FAQs about I-131 Therapy for Thyrotoxicosis

The most common side effect is hypothyroidism (underactive thyroid) which is the end result of this treatment in almost 60-70% of the cases. This is because the most of the thyroid gland has been destroyed by the beta radiation of Iodine-131.

In 90-95% of patients, a single dose of I-131 solves the purpose, ie either the patient becomes hypothyroid (underactive thyroid, chances are 60-70%) or euthyroid (normal thyroid, chances are 10-20%). However, remaining 5-10% of patients may require second dose of Iodine-131.

There is practically no effect on fertility, neither in male nor in female. However, it is advisable to plan pregnancy only after 1 year of completion of this treatment.

Yes of course. This is an out-patient treatment and in most cases the procedure takes maximum 1 hour to be completed.
